Who we are

Elevate Independence is a registered NDIS provider. This website notice should be read with any participant privacy information, service agreement or consent form provided to you when services begin.

What we collect through this website

When you use our enquiry form, we collect the information you choose to provide, which may include:

  • Your name and phone number or email address
  • Whether you are a participant, family member, friend, carer, support coordinator or referrer
  • Your suburb and the types of support you are interested in
  • Any optional message you enter

If you enquire for another person, please provide only information you are authorised to share.

Job applications

When you apply for a role through our careers page, we collect the information you enter and the documents you choose to attach. This may include your name, email, mobile number, suburb or postcode, availability, work-rights declaration, licence and transport status, relevant experience, qualifications, First Aid and CPR status, NDIS Worker Screening status, résumé and optional supporting documents.

We use this information to assess your suitability, contact you, arrange recruitment steps, make hiring decisions, meet recruitment and recordkeeping needs, and—only if you separately choose this option—consider you for suitable future roles. Required information is needed for us to assess the application; without it, we may not be able to consider you.

Access is limited to authorised people and service providers involved in recruitment. Applications and attachments are delivered to our administration mailbox using Microsoft 365. We ask for referee details and verify identity, work rights, screening, licence, insurance, training and qualifications only later when genuinely needed.

Please do not upload a passport, driver licence scan, Medicare card, tax or bank information, medical record, police check, or NDIS Worker Screening screenshot. The NSW NDIS Worker Check is verified digitally through the appropriate employer process rather than from an uploaded card or image.

We retain recruitment information only for as long as reasonably needed for the vacancy, any resulting employment relationship, our legal and recordkeeping needs, or suitable future roles where you have consented. You may ask us to access, correct or delete recruitment information, or withdraw future-role consent, using the privacy contact details on this page.

Why we collect it

We use website enquiry information to understand your request, send an automatic acknowledgement when you provide an email address, contact you, discuss suitable next steps, manage follow-up and keep an appropriate record of the enquiry and our response.

The acknowledgement confirms that we received the enquiry. It does not add you to a marketing list, confirm service availability or commit you to services.

Providing this information is optional. If you do not provide enough contact information or detail, we may not be able to respond or understand what assistance you are seeking.

Please do not include sensitive documents

Our quick enquiry form is not the right place for an NDIS number, NDIS plan, medical record, diagnosis details or other sensitive health information. If information of this kind is needed later, our team will explain an appropriate way to provide it.

How information is held and disclosed

Your website enquiry or job application is sent to our administration mailbox and held using Microsoft 365. Our website hosting and security providers process the submission so it can reach us. Access is limited to authorised people who need the information to respond, recruit, operate our systems or support service delivery.

We may also disclose information where you authorise us, where reasonably necessary to work with an agreed service provider, or where required or authorised by law. We do not sell personal information.

Security, retention and global technology services

We take reasonable steps to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. No online service can guarantee absolute security.

We keep information only for as long as reasonably needed for the enquiry, any resulting service relationship, our recordkeeping needs and applicable legal obligations.

Microsoft and our website technology providers use global infrastructure. Depending on service configuration and support arrangements, information may be processed or accessed outside Australia. Contact us if you would like current information about the providers and locations relevant to your enquiry.

Accessing or correcting your information

You may ask to access personal information we hold about you or request that inaccurate information be corrected. Email admin@elevateindependence.com.au with “Privacy request” in the subject line.

Making a privacy complaint

If you have a concern about how we handled personal information, email admin@elevateindependence.com.au with “Privacy complaint” in the subject line. Please explain your concern and how you would like us to help. We will consider it and contact you about the next steps.

Changes to this notice

We may update this notice when our website, technology providers or privacy practices change. The date at the top shows the latest update.
